Aging In Place
Helping Virginia Beach Residents Stay Safe and Independent at Home
Our Mission:
To support Virginia Beach residents in maintaining their independence, preventing falls, and reducing non-emergency reliance on Virginia Beach EMS and Virginia Beach Fire Department services. Through referrals from EMS or Fire personnel, the Aging in Place program provides a coordinated approach to improving home safety, connecting residents with resources, and offering ongoing support to help you or your loved ones feel safe at home.
A Collaborative Community Effort: The Aging in Place program is a partnership between the Virginia Beach Fire Department, Virginia Beach EMS, and the Virginia Beach Department of Public Health. Together, we are committed to supporting the health, safety, and independence of our residents.
Our Program:
The Aging in Place program is only available through referral from the Virginia Beach Fire Department or Virginia Beach EMS. If you or someone you love has been referred, here’s what you can expect:
- Referral: You may be referred to our program by Virginia Beach EMS, Fire Department, or the Virginia Beach Department of Public Health.
- Initial Contact: A representative from the Aging in Place team will contact you within 24-48 business hours after referral.
- Home Visit: We’ll schedule a convenient time for your in-home assessment. Please plan for 1-2 hours during the first visit.
- Referrals and Support: Following the assessment, we’ll recommend services, assist with obtaining durable medical equipment if needed, and arrange any necessary follow-up visits.
What we offer:
- In-Home Safety Assessment: We conduct a thorough, personalized home evaluation to identify fall risks, mobility challenges, and other safety concerns.
- Connection to Local Resources: Based on your needs, we connect you to trusted community services, including healthcare providers, home modification specialists, and other supportive programs.
- Care Coordination and Follow-Up: Based on the assessment, we may connect you to trusted local services, assist with obtaining durable medical equipment (DME), and arrange follow-ups if needed.
Why Focus on Fall Prevention?
Falls are the leading cause of injury among older adults. Through proactive measures, home modifications, and care coordination, Aging in Place aims to reduce falls, prevent injuries, and minimize unnecessary 911 calls — giving you greater peace of mind in your own home.
